Financial & Insurance

It is our goal to provide a financial option that will allow you to comfortably receive the dental services you need or desire. We are pleased to be able to offer a variety of financial options to serve your needs.

Our Financial Policy requires payment, or arrangement of payment, for services as treatment begins. For those with dental insurance, the payment due includes any estimated portion not covered by insurance (deductibles, co-payments, insurance plan exclusions or limitations, etc.). Please note that you are fully responsible for all fees charged, independent of insurance involvement. If extended payment arrangements are needed, our policy requires the use of one of our established methods:

  • Pay-as-you-go: Pay your financial obligation as treatment is rendered by cash, check, debit or credit card (Visa, MasterCard, Discover or American Express).
  • Dental Fee Courtesy: For amounts over $200, a fee courtesy is available for Payment-in-full (including estimated insurance).
  • Payment-on-Hold: Pay your financial obligation in two parts: (1) 50% paid as treatment begins and (2) final check or credit card payment held up to 30 days.
  • HealthCare PayPlan: In-office financing through pre-authorized checking account deductions or credit card installments.
  • Citi Health Card: A separate credit line available for healthcare expenses. No down payment required and up to 12 months available with no interest.
  • Care Credit: An outside financing service offering up to 12 months interest-free financing.

As treatment is recommended, a customized Financial Options form will be provided for you that further details and explains each option. Sherri or Kim will be glad to help you find a financing option that is right for you!

Additional Information:

Minors: Children under 18 must be accompanied by a parent or guardian, or have a signed parental consent form on file for treatment to be rendered. Payments for any financial obligation must be prearranged or paid in full the day of service. For children of divorced parents, the parent who authorizes treatment must also be the parent responsible for payment, or arrangement of payment, the day of service. We will not accept payment from third party claim administrators, such as attorney's, workman's compensation, or any liable corporation. As a courtesy, our office is pleased to accept the assignment of benefits from your primary dental insurance carrier. We are also unable to file medical insurance claims. We will provide any dental imformation necessary for you to submit your medical claim. We request that payment or payment arrangements be made as treatment is rendered.

Delinquent Accounts: There will be a $15.00 Non-Sufficient Fund fee for any returned check or decline of any pre-authorized payment. Account balances over 60 days old will incur a 1 1/2% per month (18% APR) finance charge. Accounts remaining in default will be subject to additional fees pertaining to the collection of the account, including attorney's fees.
